Please register on the form below for the upcoming 8-hour ServSafe® training class with ServSafe® Food Protection Manager Certification examination. (You may also register with me by telephone). Please indicate the class date and location that you are registering for in the comments section on the form below.  Please indicate the mailing address where you may receive course materials through the mail. Don't forget to select the SEND button after completing the form. The cost per each participant would be due immediately after you receive a return e-mail confirmation of your on-line registration (or after verbal confirmation when registering by telephone). You should receive your e-mail confimation within 24 hours of registering.  Please make check or money order payable to ML Food Safety Consulting LLCPlease mail payment to:  Michael Leichliter, ML Food Safety Consulting LLC, 123-A Alder Avenue, Egg Harbor Township, NJ 08234-9302. 
 
Check Policy:  If your check is dishonored or returned for any reason, this merchant will electronically debit your account for the amount of the check plus a state (NJ) non-sufficient funds (NSF) fee of $30.00 (or legal limit) plus any applicable sales tax.  The use of a check for payment is your acknowledgment of the NSF check policy.
 
Cancellation Policy:  Classes canceled by paid participant(s) or by Michael Leichliter of ML Food Safety Consulting LLC will be handled as follows: All sales are final.  The classes and any materials supplied by me are nonrefundable. Participants must contact me immediately if they will not be able to attend the class that they have paid for.  Participants may elect to send another person to attend the class in their place if they desire.  On the same note, I will immediately notify participant(s) in the event that I need to cancel a class.  If any of the scenarios arise as described above, participants will be guaranteed a seat in my next class at no additional cost.
